We are currently accepting new patient’s ages 6 months to
10 years old. If We recommend printing and completing these forms at your
convenience before your child's first appointment. If you have any problem
getting these files to print properly, please call our office and we will
gladly send or fax them to you.

Appointment
At Nacogdoches Pediatric Dentistry, we understand that you
have busy schedules to coordinate. That's why we offer convenient
appointment times that are suitable for both you and your child.
Preschool children should be seen in the morning because they are fresher
and we can work more slowly with them for their comfort. School children
with a lot of work to be done should be seen in the morning for the same
reason. Dental appointments are an excused absence. Missing school can be
kept to a minimum when regular dental care is continued.
Our office hours are Monday - Thursday 8:00 a.m. - 4:30 p.m.

time!
Appointment Policy
The scheduled appointment is reserved specifically for
your child. Any Change in this appointment affects many patients. If a
cancellation is unavoidable, please call the office at
least 24 hours in advance so
that we may give it to another patient. If notice is not given a $25 fee
will be charged to the account.
Please plan to arrive 5 minutes or more before your
scheduled appointment. This will allow time to complete any additional
paperwork and see your child on time.
If you arrive 10-15 minutes late for your appointment, you
may be asked to reschedule for the next available appointment time.
If two (2) broken/missed appointments or two (2)
cancellations without 24-hour notice occur, our office reserves the right to
NOT schedule any subsequent
appointments.
If at any time you have a question, please feel free to
ask our staff or call our office. We are here to help in any way we can. We
appreciate you entrusting your child’s dental health to us.
Insurance
We are currently accepting all
major insurances as an out of network provider. Unfortunately, we are not
accepting new Medicaid patients at this time.
Please understand that we file dental insurance as a courtesy to our
patients. We do not have a contract with your insurance company, only you
do. We are not responsible for how your insurance company handles its claims
or for what benefits they pay on a claim. We can only assist you in
estimating your portion of the cost of treatment. We at no time guarantee
what your insurance will or will not do with each claim. We also cannot be
responsible for any errors in filing your insurance. Once again, we file
claims as a courtesy to you.
We request that you provide our office with accurate information relative to
your insurance plan prior to treatment. That way, we can assess coverage to
help you best maximize your insurance benefits. Please know that you are
ultimately responsible for any services that are not covered by your
insurance company.
Payment & Financing options
If your child requires any dental treatment, we will provide you with a
detailed treatment plan and we will explain to you all estimated costs.
Because we want you to be fully informed of proposed treatment plan, please
feel free to ask us any questions you may have. We want you to be confident
that the treatment is the best course of action for your child's dental and
overall
health. We will not proceed with any treatment until we
have written consent from a child's parent or legal guardian.
Payment is due the day services are
performed. We accept cash, check, debit cards and most major credit cards.

If for any reason your account becomes 90 days past due,
it may be turned over to our Collection agency. You will be responsible for
all costs of the collection process.
